What is on it
What to ask the lab for. The wax-up, which teeth will be veneers, crowns or onlays, the prepped model and the stent.
What to tell them about your patient. The look they want, what they want to keep or lose, black triangles, guidance, spacing, midline, incisor show and shade.
What you have sent and how. Scans, photos and the prescription, so nothing is lost between the scan and the email.
